HEALTH SAVINGS ACCOUNT (HSA) SERVICES <br />Comments <br />Standard HSA services, United's affiliate will be Customer's <br />preferred HSA custodian for eligible employees' HSAs. <br />United will provide the following services in relation to those <br />HSA custodial services: <br />• Pre - enrollment brochures — one per employee. <br />• Human Resources Communication Toolkit. <br />• Provide access to bank account information through a <br />member website for account holders enrolled in health <br />plans administered by United. <br />• Facilitate the opening of HSAs for eligible employees <br />through automated batch eligibility feed to United's <br />affiliate under the terms of Exhibit E. <br />). r LEXIBLE NPENDING ACCOUNT (FSA) SERVICES <br />Service Comments <br />United will process claims for eligible Expenses (any treatment amount, service or supply paid or incurred by a Participant and <br />eligible for reimbursement under the FSA Plan and pursuant to applicable sections of IRC). Claims for reimbursement of FSA <br />Plan benefits must be submitted in a form that is satisfactory to United.
