ATTACHMENT A <br />STATEMENT OF WORK <br />I. GRANTEE RESPONSIBILITIES <br />Grantee will: <br />A. Perform professional, administrative and clerical services necessary to determine <br />eligibility, provide food benefits, and provide appropriate nutrition education and <br />counseling to qualified women, infants and children in a specified geographic area. <br />Grantee shall ensure adequate staff coverage and uninterrupted delivery of services. <br />Services shall be performed according to the statutes, rules, policies, and directives of the <br />Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) and/or <br />as directed by the United States Department of Agriculture (USDA) as referenced in this <br />Contract. During the term of this Contract, the USDA may issue regulations, instructions, <br />policies and/or d irectives, which may be incorporated into the current System Agency WIC <br />Program Policy and Procedures Manual and program rules. <br />B. Provide services within the state boundaries of Texas and in the approved clinic locations <br />described in Grantee's approved application which is incorporated herein by reference and <br />made a part of this Contract. <br />C. Assist System Agency or USDA in the collection of data that will identify benefits of this <br />nutrition intervention program and furnish financial, health, nutrition education and any <br />other special reports in a timely manner as required by System Agency's WIC Program <br />policies and procedures for the compilation of such data. <br />D. Determine eligibility of applicants through assessment of their categorical income, <br />residence and nutritional status, and provide nutrition education and counseling to eligible <br />participants. <br />E. Become and maintain designation as a Mother -Friendly Worksite in accordance with 25 <br />Tex. Admin. Code § 31.1. <br />F. Appoint a Grantee WIC Director/Supervisor. <br />G. Train Local Agency staff. The term "Local Agency" is defined at 7 CFR § 246.2. <br />H. Conduct outreach to potential participants. <br />1. Complete surveys as requested. <br />J. Complete part one, part two, or both, as follows: <br />1. Part One: Coordinate with System Agency to visit the physical address of a proposed <br />grocery store outlet and determine if a store exists. Grantee shall send System Agency <br />an email correspondence indicating findings. <br />2. Part Two: In accordance with current System Agency WIC policies and procedures, <br />coordinate with System Agency to schedule and conduct an on-site evaluation of the <br />WIC authorized vendor upon request by System Agency and complete the On-site <br />Evaluation within thirty (30) days and submit the form electronically to System <br />Agency within one (1) business day. <br />System Agency Contract No. 201;'-0497 3Y-0UI <br />v. 06.22.2016 Page 1 <br />
