DocuSign Envelope ID:COA2C376-DO83-4406-8D6C-OA6C2103A7F3 <br /> �a^Y'l�]'4Y• <br /> �,+.ra nra�,aactrrc <br /> CONTRACT NUMBER 224-156 <br /> AGREEMENT BETWEEN THE CITY OF SAN MARCOS <br /> AND CENIKOR FOUNDATION <br /> FOR <br /> OPIOID TREATMENT,AWARENESS,AND EDUCATION <br /> This Agreement ("Agreement") is made and effective as of April 1, 2024, and entered into by and between THE <br /> CITY OF SAN MARCOS, a municipality in the State of Texas ("City" or "Owner"), and Cenikor Foundation, <br /> 1901 Dutton Drive, Suite E, San Marcos,Texas 78666("the Agency"),which is a social services agency receiving <br /> grant funding from or through the City, and is effective for all purposes as of the date of the last signature to this <br /> Agreement ("Effective Date") if later than April 1, 2024. The City and Agency may also be referred to herein, <br /> individually as a"Party,"or collectively, as "Parties." <br /> The terms "Agency", "Contractor", "Vendor", and "Company" have the same meaning and shall be used <br /> interchangeably. <br /> Background This is a collaboration between the City and Agency to bring Opioid treatment, awareness, and <br /> education surrounding opiod use/issues to the San Marcos community through the opioid settlement funds. <br /> WHEREAS, on March 19, 2024,the City Council allocated$100,000 for Human Services Grants using the City's <br /> General Funds; and <br /> WHEREAS, a portion of$100,000 will be reimbursed to the Agency for non-profit activities providing public <br /> services; and <br /> WHEREAS,the Agency has legal authority to enter this agreement, and by signing this agreement,to assure the <br /> City that it will comply with all the requirements of the subaward described herein; and <br /> WHEREAS,the City's governing body has duly adopted the Resolution No. 2024-47R authorizing the City to <br /> enter into agreements necessary to implement use of the funding source and <br /> NOW, THEREFORE, it is agreed between the parties hereto that: <br /> 1. STATEMENT OF WORK <br /> A. Activities <br /> The Agency is responsible for administering the program entitled Opioid Treatment,Awareness, and Education <br /> ("the Program") for public assistance to San Marcos residents in a manner satisfactory to the City and consistent <br /> with any standards required as a condition of providing these funds. <br /> All activities funded must comply with the following statement of work: <br /> 224-156 <br /> Page 1 of 17 <br />
