OUTDOOR AREA LIGHTING RATE SCHEDULE - OAL MONTHLY RATE (continued) ments or surcharges imposed by any governmental authority which are assessed on the basis of revenues from electric service or volume of electricity purchased or sold by the city. MINIMUM CHARGE The monthly minimum charge shall be the sum of the stated charge per lamp and all applicable rate adjustments. PAYMENT The due date of the bill shall be fifteen (l5) days after issuance, and payment must be received in the electric utility office of the City by 5:00 P.M. on the due date. Any and all amounts remaining unpaid after the due date shall be assessed a charge in the amount of ten percent (10%) of the unpaid amount. SPECIAL CONDITIONS The City will furnish, operate, and maintain the facilities required for service under this rate schedule. Service at locations where existing City-owned poles and facilities are not available will be made at the discretion of the City and at an additional charge to cover the cost of installing poles and con- ductors and to cover increased cost of maintenance. The follow- ing conditions will apply to all facilities installed by the City under this rate schedule: a. The cost of repairing facilities damaged by acts of vandalism shall be billed to the Customer receiving service under this rate schedule at actual cost to the City including all appropriate overheads. The City shall, at the request of the Customer, relo- cate or change existing equipment and the Customer shall reimburse the City for any and all such reloca- tions or changes at actual cost to the City including all appropriate overheads. b. c.
