PROPONENT INFORMATION QUESTIONNAIRE: Submit the Vendor Information <br />Questionnaire (See Attachment "D ") in order to be considered responsive. <br />Failure to address Sections A - L in detail will be sufficient reason to eliminate a <br />proposal from consideration. <br />6. SUBMISSION OF PROPOSAL <br />Submit one (1) original and two (2) copies of the proposal and a CD as a pdf file (as one <br />file) to the Office of the Purchasing Manager, Municipal Building — 1s Floor, 630 E <br />Hopkins Street, San Marcos, Texas 78666 by April 28, 2011, 2:00 p.m. D.S.T. The City <br />will not accept proposals after the 2:00 p.m. submittal deadline. The sealed envelope will <br />be labeled as such: <br />DO NOT OPEN —TELECOMMUNICATION SERVICES <br />Submitted by: {Name of Proponent} <br />To the extent permitted by law, all documents pertaining to this RFP will be kept <br />confidential until a contract is awarded. No information about any proposal will be <br />released to the public until the process is complete. <br />7. EVALUATION OF PROPOSALS <br />A. EVALUATION CRITERIA: The proposals submitted in response to this RFP will be <br />evaluated by a City appointed committee. The committee will evaluate the proposals <br />based on, but not limited to, the following criteria: <br />CRITERIA POINTS <br />1. Financial stability 20 <br />2. Qualifications and competence of technical implementation staff, <br />ongoing service and support staff; Account Manager, Project Manager 35 <br />Installation Technicians, Support Technicians, Software <br />Designer /Programmer, and Subcontractor(s). <br />4. Implementation Plan - Level of clarity, comprehensiveness, and detail 15 <br />to demonstrate the ability to perform successfully and responsibly under <br />the terms of the RFP. <br />5. Pricing for services provided; both installation /activation and 20 <br />recurring monthly costs will be evaluated; as well as best overall <br />telecommunications pricing for the City. <br />19 <br />
