RFP 211 -037 <br />b) Number of technicians certified; and <br />c) Background and experience of your Operations Manger, Project Manager, <br />Service Technician(s), Equipment Installation Technician(s), Software <br />Designer /Programmer, and Cable Installation Technician(s) <br />4. Subcontractors: Any subcontractor who assists the successful Proponent in the <br />performance of this work will meet the same training and certification <br />requirements as the successful Proponent. <br />C. CUSTOMER SERVICE REQUIREMENTS: Provide its customer service number for <br />local services, the location of its customer service operation, the hours and days of its <br />normal operation (staffed by a "live" person), and a description of what happens to <br />callers when the customer service number is not "manned ". <br />D. REFERENCES: Provide references for at least three (3) organizations for which <br />they have performed and completed services similar to those described in this RFP <br />during the past three years. For each reference provide the following information. <br />1. Name, address, and telephone number of the owner's project representative; and <br />2. Scope of services. provided. <br />E. IMPLEMENTATION PLAN /SCHEDULE: Provide an implementation plan and <br />schedule (See Section 3.H). <br />F. REPAIR DOCUMENTATION: Provide repair procedure documentation (See <br />Section 31.3). <br />G. DISASTER RECOVERY PLAN: Provide a disaster recovery plan for force majeure <br />events (See Section 3.M). <br />H. REMOTE MONITORING CAPABILITIES: Provide detailed information of remote <br />monitoring capabilities of the T1 services installed at City location(s) (See Section 3.N). <br />I. MONTHLY TRAFFIC ANALYSIS: Provide a sample monthly traffic analysis report <br />(see Section 3.0). <br />J. INSTALLATION REQUIREMENTS: Provide a description of installation <br />requirements, including power, space, etc. for all equipment (See Section 3.P.). <br />K. COST PROPOSAL: Provide the Cost Proposal Form (See Attachment "A ") as part <br />of your proposal. Failure to complete and submit the provided Cost Proposal Form <br />will result in disqualification. <br />18 <br />
