RFP 211 -037 <br />5. RESPONSE TO THE REQUEST FOR PROPOSALS <br />In order to expedite the evaluation of proposals, proponents will organize their <br />proposals in the sequence provided below. At a minimum your response to this RFP <br />will conform to and include the following required items: <br />A. PROPONENT PROFILE: Provide the following background information about your <br />company: <br />1. Companv Name: Include company name, business address, and telephone <br />number. <br />2. Year Established: Year established (including former firm names and year <br />established, if applicable). <br />3. Companv Twe: Indicate the company type, i.e.; Local Exchange Company <br />(LEC), Competitive Local Exchange Company (CLEC), Local Service Provider <br />(LSP), Interexchange Carrier (IXC) or Facilities Based. (Indicate all that apply.) <br />4. Financial Data: Your latest audited financial statement and your latest unaudited <br />financial statement. The Proponent will provide the name, address and telephone <br />number of its primary bank officer and it's Certified Public Accountant. <br />B. QUALIFICATIONS: Provide the following information about your firm and any <br />proposed subcontractors. <br />1. Certification: <br />a. Reseller Service(s) Proponent: If a reseller, a Proponent will provide a copy <br />of your agreement with the respective network operator stating the level of <br />support, i.e. Silver, Gold or Platinum or other nomenclature of the reseller. <br />b. Technicians: All service and installation technicians assigned to this Project <br />will have completed certified training on the proposed service(s). A copy of <br />certificate of completions of each technician to be assigned will be provided <br />with the proposal. <br />2. Experience: Proponent will have experience in all aspects of the work specified <br />in this RFP and be able to demonstrate direct recent experience on similar <br />projects. The Proponent will own and maintain tools and equipment necessary <br />for successful installation and testing, and employ personnel who are adequately <br />trained. <br />3. Technical Capabilitv: Provide the following information on personnel: <br />a) Number of technicians in the local office; <br />17 <br />
