Financial Data: Your latest audited financial statement and your latest unaudited <br />financial statement. Q UALIFICA TIONS <br />Provide the following information about your firm and any proposed subcontractors. <br />1. Certification: <br />a. Reseller Service(s) Proponent: If a reseller, a Proponent will provide a copy of your <br />agreement with the respective network operator stating the level of support, i.e. Silver, <br />Gold or Platinum or other nomenclature of the reseller. <br />CenturyLink is not a reseller, but direct provider of the services proposed. <br />b. Technicians: All service and installation technicians assigned to this Project will have <br />completed certified training on the proposed service(s). A copy of certificate of <br />completions of each technician to be assigned will be provided with the proposal. <br />Please see attached certifications. <br />2. Experience: Proponent will have experience in all aspects of the work specified in this <br />RFP and be able to demonstrate direct recent experience on similar projects.
