INSTALLATION REQUIREMENTS <br />Provide a description of installation requirements, including power, space, etc. for all <br />equipment (See Section 3.P.). <br />At most, all sites would need adequate space for all demarcations of the network <br />requested. A site survey by CenturyLink, your current provider, shows that no <br />additional power, space, etc. is necessary to provide the proposed services. <br />K. COST PROPOSAL <br />Provide the Cost Proposal Form (See Attachment "A ") as part of your proposal. Failure to <br />complete and submit the provided Cost Proposal Form will result in disqualification. <br />Please reference Attachment A for pricing. <br />L.
