*BLACK <br />$$' �yBOX' <br />Tit$ F i 11 11 i,') RI °f.t <br />City of San Marcos Project: Conversion to IP telephones and single CS1000E <br />Black Box Network Services has made every effort to provide the City of San Marcos with <br />accurate quotes and pricing. However, the accuracy and completeness of all final quotes is <br />dependent upon the information provided to Black Box Network Services. <br />Project Change Control <br />Should this Statement of Work change due to customer requirements or unforeseen field <br />conditions, deletions and /or additions will be addressed using an installation change order "ICO" <br />to reflect the appropriate cost adjustments. After the final equipment and /or software order, <br />additions will be invoiced at current retail pricing. <br />Project Organization and Role Definition <br />Black Box Network Services will assign a Project Manager to oversee all aspects of the <br />installation. The PM assigned will be the main point of contact for the entire installation and all <br />scheduling and /or changes to this Statement of Work will be coordinated with the PM. All <br />communications should be through the Project Manager to insure a complete and smooth <br />implementation of your communications application. <br />The City of San Marcos will designate a customer contact to be Black Box Network Services <br />primary contact regarding the administration and other matters related to the project. The <br />customer contact is responsible for coordinating all on -site project activities with the Project <br />Manager, attending all necessary meetings, working together with the Project Manager to build <br />and accept the project schedule and staying abreast of the project status to ensure all City of San <br />Marcos tasks and deliverables are completed and delivered on time as per the project schedule. <br />Project Timeline and Milestones <br />This project is designed as a four phase implementation solution with installation date(s) to be <br />determined. An installation schedule that reflects specific tasks, responsibilities, and dates will be <br />finalized based on City of San Marcos input and approved dates. <br />This project requires the accurate scheduling of personnel to complete on time. It is important to <br />complete all assigned tasks as stated on the installation schedule. Any delays with task <br />completion will result in altered costs and time frames. <br />Assumptions <br />All LAN cable and switch ports will be in place and provided by the City of San Marcos. <br />All patch cords in the equipment room and from the IP telephone to the PC will be provided by the <br />City of San Marcos. <br />A power receptacle is available at each work station that needs a local power supply for the IP set. <br />Customer Responsibilities <br />Provide a single point of contact to provide information or access to the information for system <br />design and network requirements. <br />Page 14 of 18 <br />
