<br />Service Fee Report <br /> <br />Along with the 9-1-1 Financial Report, please submit a breakout by <br />telephone company by month of the service fee revenue. This revenue <br />should be broken out by landline and wireless as noted on the 9-1-1 <br />Financial Report. Any adjustments, uncollectables, and administrative <br />fees deducted and not remitted to the COG by the telephone company <br />should be noted, with the net amount equal to line 3 of the 9-1-1 <br />Financial Status Report, Net Service Fee Revenue. Other data which <br />should be included on the Service Fee Revenue Report is number of <br />lines billed, month the TELCO collected the money. <br /> <br />Caoital Recovery <br /> <br />The Capital Recovery Beginning Balance Line should be reflected on <br />your September (or Sep- Nov quarterly) report as the prior fiscal year's <br />ending balance for Capital Recovery. This figure will remain constant <br />the entire fiscal year. <br /> <br />Capital Recovery Eamings represents any interest your Capital <br />Recovery Account earned. Please note that this amount does NOT <br />show up on Line 4. Interest Revenue (which is interest from the service <br />fee money account). <br /> <br />Recovery Revenue Requested represents the contribution for the <br />current period to the Capital Recovery Account. This figure should <br />equal the detail of Capital Recovery on the County Level Worksheet. <br />Please note the current contribution will appear on Lines 20 and 28 of <br />the 9-1-1 Financial Report. <br /> <br />Capital Recovery Expenditures represent expenditures out of your <br />Capital Recovery account for the current period. If funds were spent, a <br />description must be included on the Performance Report. Line 29 on <br />the 9-1-1 Financial Report is the only place on the Financial Report <br />where expenditures from the Capital Recovery Account will be shown _ <br />They are not listed on the County Level Worksheet. <br /> <br />Poo/Funds <br /> <br />Please exhaust all Pool Funds before requesting service fee or <br />surcharge for Addressing. <br /> <br />Reoortinq Periods <br /> <br />Please assure that the Addressing Financial Reports have consecutive <br />time periods.
