<br />DOCUMENT NO. 7460022381-01 <br />ATTACHMENT NO. 01 <br /> <br />PERFORMING AGENCY: CITY OF SAN MARCOS <br /> <br />RECEIVING AGENCY PROGRAM: BUREAU OF CHILDREN'S HEALTH <br /> <br />TERM: August 01,2000 THRU: August 31,2001 <br /> <br />SECTION I. SCOPE OF WORK: <br /> <br />RECEIVING AGENCY will assist in the initiation, implementation, and maintenance of a water <br />fluoridation system operated by PERFORMING AGENCY for the dental health benefit of the <br />population served by PERFORMING AGENCY, PERFORMING AGENCY shall purchase, <br />install, operate, and maintain equipment required to accomplish the purpose as set forth in the <br />equipment specifications, general specifications, approved vendor lists, drawings and equipment <br />component lists prepared by RECEIVING AGENCY and provided to PERFORMING AGENCY. <br />Activities shall be performed and reports submitted in accordance with the following: <br /> <br />· RECEIVING AGENCY rules and program guidelines as approved by RECEIVING <br />AGENCY and outlined in the attached Exhibit A, Texas Department of Health, Oral <br />Health Services, Texas Fluoridation Program; and <br /> <br />· "Engineering and Administrative Recommendations for Water Fluoridation, 1995," <br />Centers for Disease Control and Prevention (CDC) Publication, Morbidity and Mortality <br />Weekly Report, Volume 44, No. RR-13, which is adopted by reference as a part of this <br />contract. <br /> <br />PERFORMING AGENCY shall purchase and install initial (and any subsequent replacement) <br />fluoridation equipment in strict compliance with the equipment specifications, general <br />specifications, approved vendor lists, drawings and equipment component lists prepared by <br />RECEIVING AGENCY and incorporated by reference into and made part of this contract. Any <br />variations from PERFORMING AGENCY's equipment specifications, general specifications, <br />approved vendor lists, drawings and equipment component lists must have RECEIVING <br />AGENCY's prior written approval. PERFORMING AGENCY shall use equipment and supplies <br />for fluoridation purposes only. <br /> <br />PERFORMING AGENCY shall assure that at least one (1) operator of PERFORMING <br />AGENCY's water system attends a RECEIVING AGENCY sponsored fluoridation operator <br />training course prior to or within six months of installation completion and as necessary <br />subsequently to maintain at least one (1) RECEIVING AGENCY trained operator in <br />PERFORMING AGENCY's employ. <br /> <br />A TT ACHMENT - Page 1 <br />
