<br />PERFORMING AGENCY shall operate the fluoridation equipment so as to add fluoride chemical <br />to the water in an amount sufficient to adjust the natural fluoride level to the optimal level of 0.8 <br />parts per million. <br /> <br />PERFORMING AGENCY shall perform onsite monitoring of fluoride level in treated water each <br />normal workday and maintain a permanent record of those readings for periodic RECEIVING <br />AGENCY onsite review or for submission, if requested by RECEIVING AGENCY. <br /> <br />PERFORMING AGENCY shall submit water samples to RECEIVING AGENCY for fluoride <br />level analysis once a month or with less frequency as may be specified in writing by RECEIVING <br />AGENCY. <br /> <br />PERFORMING AGENCY shall assure RECEIVING AGENCY of the continuation of fluoridation <br />beyond the contract term by continuing to submit monthly water samples to RECEIVING <br />AGENCY for fluoride level analysis. <br /> <br />PERFORMING AGENCY shall provide written justification to RECEIVING AGENCY if the <br />project has not commenced within thirty (30) days after the execution of the contract. If <br />PERFORMING AGENCY's project has not commenced within sixty (60) days after execution of <br />the contract, RECEMNG AGENCY may cancel this contract for noncompliance according to the <br />TERMINATION Article of the General Provisions. <br /> <br />PERFORMING AGENCY shall comply with Part A, Title XIX, Public Health Service (PHS) Act, <br />as amended, 42 USC ~ 300w, et seq, (Preventive Health and Health Services Block Grant). <br /> <br />PERFORMING AGENCY shall provide services to 39.300 unduplicated clients who live or <br />receive services in the following county(ies)/area: Hays, <br /> <br />SECTION II. SPECIAL PROVISIONS: <br /> <br />General Provisions, Intellectual Property Article, is not applicable to this Attachment. <br /> <br />General Provisions, Standards for Financial and Programmatic Management Article, Item C, <br />and last two paragraphs, are not applicable to this Attachment. <br /> <br />General Provisions, Allowable Costs and Audit Requirements, IS revised to include the <br />following: <br /> <br />Funds awarded under this Attachment that are not utilized for a specific fluoridation cost identified <br />in PERFORMING AGENCY's equipment specifications, general specifications, drawings and <br />equipment component lists may be used to offset other allowable fluoridation-related costs with <br />RECEIVING AGENCY's prior written approval. <br /> <br />A IT ACHMENT - Page 2 <br />
