<br />requested documentation or information requested within this RFP is provided to the City. <br /> <br />M. SYSTEM APPROVAL: Upon completion of survey, the City and CCG will submit a I <br />letter to proponent detailing any items that may require further attention by proponent prior <br />to acceptance of services. Any changes and/or corrections which are found to be required <br />as a result of this testing will be completed by proponent within two weeks of receipt of <br />letter from the City and CCG outlining items required for completion prior to acceptance. <br />Final acceptance is contingent upon satisfactory proponent demonstration of completion of <br />all installation requirements. Upon successful completion of all items, the City and CCG <br />will issue a letter of acceptance stating specific acceptance date, which will be governing <br />date for all related RFP acceptance period items, and will be approval for final <br />payment by client to proponent. <br /> <br />N. NOTIFICATION: Prior to commencing work, the successful proponent will notify the <br />City representative prior to commencing work. Once installation has commenced, the <br />successful proponent's technician will not leave the installation without contacting the City <br />representative and informing them of status of job, reason for leaving, and expected return. <br /> <br />O. MAINTENANCE AND ONGOING SERVICE: The successful proponent will maintain <br />all on-site, cable, materials, and equipment for the duration of the contract. Equipment <br />includes Decoder Service Unit ("DSU")/Coder Service Unit (s) ("CSU"), Drop & Insert <br />Muxes, and any other equipment needed for the total turnkey installation of service(s). The <br />successful proponent will be responsible for and will incur all costs for any material, cable <br />or equipment repairs. If it is documented during the contract period that any specific I <br />equipment, component or cable requires maintenance deemed excessive by manufacturer <br />standards, proponent will replace equipment or cable at no cost to the City. <br /> <br />The successful proponent will provide all maintenance at no cost to the City at the City's <br />location specifically related to on-site equipment and cable installed by the successful <br />proponent. All travel expenses incurred in providing client on-site maintenance will be the <br />sole responsibility of the proponent. The successful proponent will provide all maintenance <br />test equipment, replacement parts, and qualified personnel to maintain and service client <br />equipment and cable. The successful proponent is responsible for any costs incurred by <br />the City in an attempt to enforce these terms, inclusive of court costs and attorney fees, <br />should the proponent fail to honor. <br /> <br />P. SYSTEM FAILURES: The following outlines the guidelines, procedures, and definitions <br />required in the event of system failures. The successful proponent will be responsible for <br />periodic notification of City's representative concerning all phases regarding status of <br />system repair. The successful proponent will provide a system certified technician (the <br />"technician") for all system failures defined below. If the successful proponent or its <br />technician are required to leave the City site for any period prior to completion of repair, the <br />City's representative will be notified of the reason for departure and the estimated time of <br />return. If the proponent is delayed, proponent will notify the City's representative. <br /> <br />a. A Major Service Failure is defined as any of the following: <br />- 8 - <br /> <br />I <br /> <br />1.
