Attachment A Continued Baker-Aielden & Associates, Ine. lune 3, 1999 PSI Proposal No. 301-9084 Page Three of Seven It is proposed that the fee for the performance of the above outlined services be determined on a unit price basis, in accordance with the attached Schedule of Services and Fees, and that the work be performed pursuant to the PSI General Conditions. Copies of the PSI Schedule of Services and Fees and General Conditions are enclosed herewith and incorporated into this proposal. On the basis of the estimated quantities and the Schedule of Services and Fees, it is estimated that the total fee for this subsurface exploration and geotechnical investigation will be $4,450.00. Our fee covers the work needed to present our findings and pavement recommendations in a report form. Not included are reviews of drawings, preparation of construction specifications, special conferences and any other work requested after submittal of our report. We estimate that this geotechnical study will take two (2) to three (3) weeks to complete following the completion of the drilling operations. PSI will proceed with the investigation based upon verbal authorization. PSI will issue the report of its findings only after receipt of a signed copy of this proposal intact. When returning the proposal, please complete the attached Project Data Sheet so that PSI may best serve your project.
