DEPARTMENT OF STATE HEALTH SERVICES INTERLOCAL COOPERATION CONTRACT This Contract, number 2013- 042558 -001, is entered into by and between the Department of State Health Services (DSHS or the Department), an agency of the State of Texas, and the CITY OF SAN MARCOS, a municipal agency charged with the exclusive management and control of the water system of the City of San Marcos, (collectively, the Parties). 1. Purpose of the Contract. The purpose of this Contract is to set out the roles and responsibilities of DSHS and the CITY OF SAN MARCOS in the cooperative effort of the purchase and installation of a water fluoridation system for the oral health benefit of the population served by DSHS. The Parties agree as follows: 2. Roles and Responsibilities of DSHS DSHS will be responsible for: a. Purchasing the equipment as determined by DSHS and providing it to the CITY OF SAN MARCOS, or causing it to be provided to the CITY OF SAN MARCOS. for use in a water fluoridation system operated by the CITY OF SAN MARCOS for the oral health benefit of the population served by DSHS. DSHS will retain ownership of the equipment for its useful life. The equipment being purchased by DSHS is the following: PolyProcessing- 6150 - gallon, single wall cross link polyethylene storage tank, overflow fitting, a -vent, ladder, and float sight gage. The serial number will be incorporated into the contract within one month of delivery. The price of the equipment is $16,412.00.
