The CITY OF SAN MARCOS shall be responsible for: <br />a. Installing the fluoridation equipment within 45 days of the CITY OF SAN MARCOS's <br />receipt of the equipment, and operating, repairing, and maintaining equipment required to <br />operate and support the water fluoridation system as set forth in the equipment <br />specifications, general specifications, approved vendor lists. drawings, and equipment - <br />component lists prepared by DSHS and provided to the CITY OF SAN MARCOS. <br />b. Accepting corresponding responsibility for, and installing, the fluoridation equipment <br />provided to the CITY OF SAN MARCOS in strict compliance with the equipment <br />specifications, general specifications, approved vendor lists, drawings, and equipment - <br />component lists prepared by DSHS and incorporated by reference into and made part of <br />this Contract. Any variations from the CITY OF SAN MARCOS's equipment <br />specifications, general specifications, approved vendor lists, drawings, and equipment - <br />component lists shall require the prior written approval of DSHS. <br />c. Using equipment and supplies provided by DSHS to the CITY OF SAN MARCOS for <br />fluoridation purposes only. <br />d. Assuring that at least one (1) operator of the CITY OF SAN MARCOS's water system <br />attends a DSHS sponsored fluoridation operator training course prior to or within six <br />months of installation completion and any subsequent DSHS - sponsored fluoridation <br />operator training deemed necessary by DSHS to maintain at least one (1) DSHS trained <br />operator in the CITY OF SAN MARCOS's employ. <br />e. Operating the fluoridation equipment so as to add fluoride chemical to the water in an <br />amount sufficient to adjust the natural fluoride level of the water to the optimal level of 0. <br />7 parts per million. <br />f. Performing onsite monitoring of the fluoride level in treated water each normal workday <br />and maintaining a permanent written record of those readings for periodic DSHS onsite <br />review and for submission to DSHS, as requested by DSHS. <br />g. Assuring DSHS of the continuation of fluoridation by submitting water samples to DSHS <br />for fluoride level analysis once a quarter, or as specified in writing by DSHS, as long as <br />the CITY OF SAN MARCOS continues to operate the fluoridation equipment. Samples <br />should be marked 'Specimen Delivery' and mailed via the U.S. Postal Service as follows: <br />Specimen Receiving <br />Laboratory Services Section, MC 1947 <br />Department of State Health Services <br />PO Box 149347 <br />Austin, TX 78714 -9347 <br />h. Assuring DSHS of the continuation of fluoridation beyond the Contract tern by <br />continuing to submit quarterly water samples to DSHS for fluoride level analysis. <br />i. Notifying the DSHS contact in Section 1 1 below in writing, via e -mail or fax, of <br />installation of fluoridation equipment no later than forty -five (45) days following the <br />CITY OF SAN MARCOS's receipt of equipment. The CITY OF SAN MARCOS shall <br />maintain an inventory of equipment and property and submit an annual cumulative report <br />of the equipment and other property on Form GC -11 (Contractor's Property Inventory <br />Report) to the Department's Contract Oversight and Support Section, Karl Code 1326, <br />P.O. Box 149347, Austin, Texas 78714 -9347, no later than October 15t of each year. <br />-2- <br />
