<br /> -5- <br /> Family member: The spouse and any dependent children of any <br /> official or candidate. <br /> Gift: An economic benefit received other than as <br /> compensation, but not including campaign contributions <br /> reported as required by state law. <br /> Identification: For an individual, the person's name, street <br /> address, city and state; for any entity other than an <br /> individual, the name, address, city and state of the <br /> entity's principal location or place of business, the type <br /> or nature of the entity's principal location or place of <br /> business, the type or nature of the entity, the date on <br /> which it came into existence, the state of incorporation, if <br /> any, and the names of the partners or officers of the <br /> entity. <br /> Income: Economic benefit received. <br /> Source of income: Any business entity, employment, investment <br /> or activity which earned or produced income, including <br /> interest, dividends, royalties or rents. <br /> Sec. 2-82. Financial disclosure statement required. <br /> ( a) Between March 15th and April 30th of each year, every city <br /> official shall file a sworn financial disclosure statement with <br /> the city secretary reflecting the financial situation of the <br /> official as of December 31st of the previous year and the <br /> official's financial activity between January 1st to December <br /> 31st of the previous year. <br /> (b) A newly appointed or elected city official shall file a <br /> sworn financial disclosure statement with the city secretary <br /> within thirty (30) days from the date the position is assumed. <br /> The statement shall reflec t the financial condition <br /> as of that date and financial activity for the previous twelve <br /> (12) months. <br /> ( c) A candidate shall file a sworn statement with the city <br /> secretary not later than ten ( 10) days from the date the <br /> candidate files with the city secretary an application to be <br /> placed on the official ballot. This statement shall include the <br /> following: <br /> (1) A list of any financial interest the person has, direct or <br /> indirect, in real property located within the corporate <br /> limits of the city or within the extraterritorial <br /> jurisdiction of the city. <br /> ( 2) A list of any financial interest the person has, direct <br /> or indirect, in any business entity located in the city <br /> or its extraterritorial jurisdiction or any business <br /> entity doing business with the city. <br /> ( 3) The financial interests listed by the person shall include <br /> those held at any time during the twelve (12) months <br /> preceding the date of filing for office, and shall include <br /> any interest held by the candidate or a family member of <br /> the candidate. <br /> (d) Any person filing a statement required by this section may <br /> include additional time periods. <br />
