<br /> d. A written evaluation and recommendation will be <br /> provided to the City if the participant employee was <br /> referred to Riverwood by the City. <br /> e. If the participant employee was referred to Riverwood <br /> by the City, Riverwood will notify the City when said <br /> participant employee began treatment and, on a weekly <br /> basis, whether the participant employee is continuing <br /> to participate in the 6/60 Program or other Ri verwood <br /> services or referrals. <br /> f. Riverwood will provide a physician evaluation on <br /> readiness to return to work for participant employees <br /> who are using sick leave or vacation leave. <br /> 4. Riverwood shall receive the sum of $2,100.00 for each <br /> participant employee who is enrolled hereunder in the 6/60 <br /> Program, it being understood that each participant employee shall <br /> utilize existing group medical insurance benefits to cover said <br /> cost to the extent available, with the City paying such part of <br /> the sum of $2,100.00 not otherwise paid. <br /> 5. Riverwood shall indemnify and hold the City harmless <br /> from and against any and all claims, actions, losses, and damages <br /> caused by or arising out of the performance, failure to perform, <br /> or breach of any of Riverwood's obligations under this Agreement. <br /> 6. This Agreement shall commence on the date of this <br /> Agreement and shall continue for a one-year period. At any time <br /> during the term of this Agreement, either party may cancel this <br /> Agreement by giving the other party written notice of <br /> cancellation thirty (30) days prior to the effective date of <br /> -3- <br />
