• Tracking system for property and auto losses - track subrogation, sent <br />to DA, TML and amount received. <br />N <br />NEED MORE INFO <br />• Tracking system for workers compensation of incurred cost. Monthly <br />receive updated files in excel and it would be nice to import into system. <br />N <br />NEED MORE INFO <br />• We use web based system with carrier to complete state promulgated <br />First Report of Injury (DWC001)and Wage Statement. Currently have to look up info in <br />system and type into system. Would like ability to populate form with HRIS info. Is <br />there any type of dynamic field linking between Tyler and other systems (i.e. Quick <br />Fields)? <br />N <br />NEED MORE INFO <br />Reporting <br />■ Demonstrate standard system reports <br />■ Worker's Comp Reports: <br />1. Subject wages (all reduced to straight time) without calculation <br />2. Calculated premium <br />3. Detailed by employee including calculations <br />4. Total number of employees by classification code <br />5. Generate reports monthly, quarterly, annually <br />6. Report to include required fields for annual audit <br />7. Hours worked by employee by department <br />■ Motor Vehicle Report run monthly on the Driver's Licenses of employees <br />who fall under the City's driving policy. The report must meet the file layout of the DPS: <br />NEED MORE INFO THOUGH ON <br />Excel format with employee first and last name, birthdate, Driver's License number <br />Y <br />FORMAT. <br />■ Employee Group Report- report that can be used to send birthday cards <br />to employees; report should pull all employees except temporaries and provide the first <br />and last name, department, month and day of birth (exclude the year). Report is run <br />monthly. <br />Y <br />■ Demonstrate standard system reports <br />Y <br />• Provide sample reports to meet requirements provided in RFP. <br />Y <br />• Demonstrate report writer — do we need to provide scenarios? <br />Y <br />• EEO 4 <br />Y <br />• PPACA includes many reporting requirements which aren't yet defined. <br />Will you provide system enhancements by compliance dates to meet these reporting <br />requirements? <br />N <br />NEED MORE INFO. <br />
