<br /> 127 <br /> LH <br /> SPECIAL SUPPLEMENTAL FOOD PROGRAM <br /> FOR HOHEN, INFANTS AND CHILDREN (HIC) <br /> <br /> AGREENENT <br /> In order to effectuate the Special Supplemental Food Program for Homen, <br /> Inf3nts and Children, hereinafter called WIC, of the United States Department <br /> of Agricclltun:-. (USDA), the !exas Department of Health, hereinafter calle¿ <br /> :>tate, and <br /> CITY OF SAN MARCOS (36) <br /> the approved WIC Project, hereinafter called Project agree as [ollO\,;s: <br /> ~. <br /> For and in consideration of the payments and commitments ;::0 be maòe and <br /> performed by State:, froject agrees and promises to: <br /> A. Perform professional, administrative and clerical services necessary <br /> to provide special supplemental foods to qualified women, infants and <br /> children in a specified geographic area as stated in the approved <br /> application according to the terms and specifications of such program <br /> as set out in State and/or USDA regulations, instructions, policies, <br /> and/or directives that will be issued to Project as they are formulated, <br /> and appear in the State WIC Policy and Procedure manual; <br /> B. Assist in the collection and evaluation of data which will medically <br /> identify benefits of this nutrition intervention program and to furnish <br /> financial, dietary, medical, nutrition education and any other special <br /> reports as are requested by the State for the compilation of the foresaid <br /> data; <br /> C. Determine eligibility of applicants. register participants, collect <br /> data, conduct measurements, provide nutrition education, and maintain <br /> records as may ~e required by State and/or USDA Regulations for this <br /> program as iden~ified in the State Policy and Procedure manual; <br /> D. Provìùe a tood vouc~er delivery system through the issuance for pre- <br /> numbered food" oud,eri'o to qualified participants who will use such <br /> vouchers to ort"-lD. che specified food items from participating vendors; <br /> to maintain coffiplete accountability of all food vouchers received from <br /> the State. and to be held financially responsible for illl unaccounted <br /> food vouchers; <br /> E. Submit a State of Texas Purchase Voucher, AG-37. month~y or as needed. to <br /> claim reimbursable costs; Administrative and Nutrition E~ucation costs <br /> must he b~cken out on the face of the voucher. Al.3o, in accordance with <br /> the Uniform Grants a:id Contract Hanagement Stand:lH!S, submit to;",1e State <br /> quart~rly a Fincilcial Status Report, State Supplemental Form 26Sa, of total <br /> Project Administrative and Nutrition Education expenses Dy the 20th of <br /> the month following the report month. A Schedule of Equipment Purchases, <br /> AC 78-6, must also be submitted if inventoried equipment was purchased <br /> during the quarter. <br /> F. Shall submit as a final annual close-out report a Financial Status Report <br /> State Supplemental Form 269. for total Project Administrative and Nutrition <br /> Education expenses no later than December 30, 1984, and if necessary. a <br /> State of Texas Purchase Voucher for all costs that have not been reimbursed. <br /> G. Shall submit monthly performance data which includes: Report of Program <br /> Operations, AC 80-1, Report of Card Sequence Used. AC 80-3, and Cards <br /> Voided. AC 80-4. Lost/Stolen Report, AC 78-10, shall be submitted as <br /> necessary. Also on an annual basis. submit a Racial/Ethnic Group <br /> Participation Form, FNS-19l, by the seventh working day of October. <br />
