Contractor cannot agree to the substitution of the Project Representative, the City may terminate <br /> this agreement in accordance with Article 7. The Contractor's Project Representative shall <br /> assign all necessary personnel including Case Managers and Intake Specialists and establish <br /> Goals and Performance Measures. The Contractor will provide Case Management and Pre- <br /> Construction Services for up to 80 applicant households that submitted an application for one of <br /> the CDBG-DR Housing Programs. <br /> 1.1 TASK 1 - HOUSING CASE MANAGEMENT SERVICES <br /> AGENCY RELATIONS <br /> The Contractor will: <br /> 1.1.1 Using the appropriate Policy and Procedures Manual: Single Family Owner-Occupied <br /> Rehabilitation and Reconstruction Housing Program Policy and Procedures Manual or the Rental <br /> Property Housing Reconstruction Policy and Procedures Manual assist the City with Applicant <br /> Relations by securing the necessary personnel and equipment to be able to process applications <br /> in-person and over the phone within fifteen(15) days of the contract start date in accordance <br /> with Housing Programs Guidelines, policies, procedures, and business processes involving <br /> determination of eligibility, benefits and a prioritization of applications. <br /> 1.1.2 Assist applicants with application completion and submission. <br /> 1.1.3 Perform in accordance with the City's program guidelines reviews of application and <br /> determining eligibility. <br /> 1.1.4 Safeguarding Sensitive Information <br /> Maintain the privacy of applicant information as allowed under the Texas <br /> Public Information Act, Chapter 552 of the Texas Government Code. <br /> Safeguarding Sensitive Information. The Contractor shall comply with The <br /> Privacy Act of 1974, 5 U.S.C. § 552a, The Freedom of Information Act 5 <br /> U.S.C. § 552, and Section 208 of The E-Government Act regarding the <br /> disclosure of information about clients. Compliance for CDBG-funded projects <br /> is also spelled out in 24 C.F.R. § 5.212. <br /> i. The collection, maintenance, use, and dissemination of SSN's, <br /> Employer Identification Numbers (EINs), any information <br /> derived from SSN's and EINs, and income information under <br /> this subpart shall be conducted, to the extent applicable, in <br /> compliance with the Privacy Act and all other provisions of <br /> Federal, State, and local law. <br /> ii. All assistance applicants shall be provided with a Privacy Act <br /> notice at the time of application. <br /> iii. All records, both electronic and paper copies, shall be <br /> maintained in systems that have the appropriate administrative, <br /> technical, and physical safeguards to protect the information, <br /> however current. <br /> 3 <br />
