CERTIFICATIONOFELIGIBILITY <br />BysubmittingaproposalinresponsetotheInvitationforBids,theContractorcertifiesthatatthetime <br />ofsubmission,he/she/itisnotlistedonthegovernmentwideexclusionsintheSystemforAward <br />Management(SAM),inaccordancewiththe0MBguidelinesat2CFR180thatimplementExecutive <br />Orders12549(3CFRPart1986Comp.,p189)and12689(3CFRpart1989Comp.,p235),"Debarment <br />andSuspension". <br />1) Intheeventofplacementonthelistbetweenthetimeofbid/proposalsubmissionandtimeof <br />contractaward,thebidder/proposerwillimmediatelynotifytheCity. <br />2) Contractorcertifiesthatitssubcontractorsarenotpresentlydebarred,suspended,orproposedfor <br />debarment,declaredineligibleorvoluntarilyexcludedfromparticipationinanystateorfederal <br />program. <br />3) PlacementofContractoronthefederalgovernment'slistofsuspended,ineligible,ordebarred <br />contractors,falsecertification,orfailuretonotifyCityasrequiredmayresultinCity'sterminationof <br />thisContractfordefault. <br />4) Contractorwillfurnishacopyofthecertificationinaccordancewith24C.F.R.Part24(Debarment <br />andSuspension).TheContractorandallsubcontractorswillbeactiveandnotdebarredonthe <br />website,www.sam.gov.andprovideacopyofthecertificationtotheCitybeforetheentityperforms <br />workunderthiscontract. <br />TheContractormustensurethatawardsarenotmadetoanypartywhichisdebarredorsuspendedoris <br />otherwiseexcludedfromorineligibleforparticipationinFederalassistanceprogramsunderExecutive <br />Order12549,"DebarmentandSuspension"andtheUAR.Contractorwillhaveeachcontractorand <br />subcontractorcompleteacontractor <br />eligibilityforminaformatthatisprovidedorapprovedbytheCity. <br />Thisformwillprovidetheinformationnecessarytoverifycontractoreligibility. <br />E. NONCOLLUSIONCERTIFICATION <br />TheContractorcertifiesthat,ifaproposalwasprovidedthatresultedinacontract,thatproposalwasmade <br />withoutcollusionwithanyotherperson,firmorcorporation. <br />F.
