Sec. 90.201. Application. An application for an operating permit required i_n section 90.200 shall be filed with the city clerk upon forms provided by the city. The application shall contain the following information: • (1) The name and address of the applicant. (2) The financial status of the applicant, including the amounts of all unpaid judouents against each applicant and the nature of the transaction or acts giving rise to the judgments. (3) If the applicant is a partnership, the information in subsections (1 and (2) of this section shall be provided for each partner. (4) if the applicant is a corporation, the name and place of residence of all of the officers of the corporation, a duly certified copy of its charter and bylaws, the amount of capital stock, the character and value of its assets, its liabilities and the security therefor and, if the corporation is a foreign corporation, a duly certified copy of its certificate of authority to transact business in the state. Sec. 90.202. Amount of insurance. Each holder of an operating _permit for the operation of a pedicab business shall at all times during t term of the operating pennit maintain a policy of liability insurance issued by an insurance company authorized to do business in the state on each and every motor vehicle operated under the temis of the operating permit in the amount of $100,000.00 for bodily injury to any one person: in the amount of 5300,000.00 for injuries to more than one person which are sustained in the same accident; and $100,000.00 for property damage resulting from any one accident. The policies shall be approved by the city attorney and filed with the city secretary. The policies of insurance shall not be cancelled or surrendered except after ten days' written notice to the city secretary. Failure of the operating permit holder to procure and file the policies of insurance as required by this section or failure to keep the policies in force shall immediately forfeit and make the operating permit null and void, and all rights thereunder shall at once cease. • Sec. 90.203. Fares (a) A pennit holder shall file with the city clerk a schedule of fares to be char,ed for services provided. A holder shall notify the city clerk no less than forty-eight (48) hours before any ges in the fare schedule filed by the holder. (b) The schedule of fares shall be conspicuously posted on each pedicab. (c) The use of anv type of meter or measuring device to calculate rate of fare is prohibited. Sec. 90.204. Vehicle requirements (a) In addition to any requirement established by the director, each pedicab operated pursuant to this chapter must be equipped with the following: (1) A headlig lt; (2 Two (2) taillights; (3) Turn si:)nals; (4) Rear and side reflective devices; (5) A braking system; (6) A horn; (7) A rearview mirror; (8) A safety flag; and (9) A slow moving vehicle emblem. (b) The name of the company operating a pedicab and the telephone number of that company shall be conspicuously posted on each pedicab. • Sec. 90.205. Service area
