Sec. 90.201. Application. <br />An application for an operating permit required i_n section 90.200 shall be filed with the city clerk upon <br />forms provided by the city. The application shall contain the following information: <br />• (1) The name and address of the applicant. <br />(2) The financial status of the applicant, including the amounts of all unpaid judouents against each <br />applicant and the nature of the transaction or acts giving rise to the judgments. <br />(3) If the applicant is a partnership, the information in subsections (1 and (2) of this section shall be <br />provided for each partner. <br />(4) if the applicant is a corporation, the name and place of residence of all of the officers of the <br />corporation, a duly certified copy of its charter and bylaws, the amount of capital stock, the character <br />and value of its assets, its liabilities and the security therefor and, if the corporation is a foreign <br />corporation, a duly certified copy of its certificate of authority to transact business in the state. <br />Sec. 90.202. Amount of insurance. <br />Each holder of an operating _permit for the operation of a pedicab business shall at all times during t <br />term of the operating pennit maintain a policy of liability insurance issued by an insurance company <br />authorized to do business in the state on each and every motor vehicle operated under the temis of the <br />operating permit in the amount of $100,000.00 for bodily injury to any one person: in the amount of <br />5300,000.00 for injuries to more than one person which are sustained in the same accident; and <br />$100,000.00 for property damage resulting from any one accident. The policies shall be approved by <br />the city attorney and filed with the city secretary. The policies of insurance shall not be cancelled or <br />surrendered except after ten days' written notice to the city secretary. Failure of the operating permit <br />holder to procure and file the policies of insurance as required by this section or failure to keep the <br />policies in force shall immediately forfeit and make the operating permit null and void, and all rights <br />thereunder shall at once cease. <br />• Sec. 90.203. Fares <br />(a) A pennit holder shall file with the city clerk a schedule of fares to be char,ed for services <br />provided. A holder shall notify the city clerk no less than forty-eight (48) hours before any ges in <br />the fare schedule filed by the holder. <br />(b) The schedule of fares shall be conspicuously posted on each pedicab. <br />(c) The use of anv type of meter or measuring device to calculate rate of fare is prohibited. <br />Sec. 90.204. Vehicle requirements <br />(a) In addition to any requirement established by the director, each pedicab operated pursuant to this <br />chapter must be equipped with the following: <br />(1) A headlig lt; <br />(2 Two (2) taillights; <br />(3) Turn si:)nals; <br />(4) Rear and side reflective devices; <br />(5) A braking system; <br />(6) A horn; <br />(7) A rearview mirror; <br />(8) A safety flag; and <br />(9) A slow moving vehicle emblem. <br />(b) The name of the company operating a pedicab and the telephone number of that company shall be <br />conspicuously posted on each pedicab. <br />• Sec. 90.205.
