<br />each employee's attendance based upon information <br /> <br />furnished by the operating departments. <br /> <br />2. <br /> <br />Excused Absences - Excused absences are defined as <br /> <br />those which have been outlined in the City Personnel <br /> <br />Policies and approved by the employee's supervisor. <br /> <br />Examples include: <br /> <br />paid sick leave, vacation, <br /> <br />holidays, emergency leave, work related injury leave, <br /> <br />and other types of approved time off. <br /> <br />To be eligible <br /> <br />for an excused absence, an employee must contact <br /> <br />his/her supervisor no later than one hour after the <br /> <br />start of the employee's regularly scheduled starting <br /> <br />time. <br /> <br />The reason for the absence and the date and <br /> <br />time the employee expects to return to work must be <br /> <br />reported at this time. <br /> <br />This one (1) hour provision <br /> <br />may be waived by the supervisor due to extenuating <br /> <br />circumstances. <br /> <br />3. <br /> <br />Unexcused Absences - Unexcused absences are defined <br /> <br />as follows: <br /> <br />a. <br /> <br />Tardiness. <br /> <br />b. <br /> <br />Unverified sick time. <br /> <br />c. <br /> <br />Unauthorized time off. <br /> <br />d. <br /> <br />Failure to call in within one hour after <br /> <br />e. <br /> <br />regularly scheduled starting time. <br />Excessive unpaid sick time or lost time. <br /> <br />While time lost due to unpaid verified illness <br /> <br />-40- <br />
