<br />or accident is considered an excused absence, <br /> <br />excessive time lost may be an indication that <br /> <br />the employee is not well enough to perform the <br /> <br />job. <br /> <br />f. <br /> <br />An employee may be required to furnish a <br />doctor/hospital statement to the supervisor <br /> <br />verifying an illness or accident under any of <br />the following circumstances in order for the <br /> <br />absence to be considered excused: <br /> <br />(1) <br /> <br />When illness or accident caused the <br /> <br />employee to be hospitalized. <br /> <br />{2) <br /> <br />When the absence was for more than <br /> <br />three (3) consecutive work days. <br /> <br />(3) <br /> <br />When the absence occurred on a <br /> <br />regularly scheduled work day preceding <br />or following a holiday or regularly <br /> <br />sched~led vacation day. <br /> <br />(4) <br /> <br />When the Supervisor feels that the <br /> <br />employee's return to work may pose a <br /> <br />health hazard to others. <br /> <br />(5) <br /> <br />When the employee has a record of <br />excessive use of paid or unpaid sick <br /> <br />leave. <br /> <br />4. <br /> <br />ReDeated Unexcused Absences - Employees repeatedly <br />incurring unexcused àbsences are considered absence <br /> <br />-4l- <br />
