<br />retain any unused sick leave or vacation leave balance. <br /> <br />In cases involving sickness or injury, an employee must <br /> <br />first use sick leave before being considered for a leave <br /> <br />of absence. <br /> <br />If an employee requests a leave of absence <br /> <br />for more than thirty (30) days, the Department Director <br />may require that the employee use accrued vacation leave <br /> <br />before being considered for a leave of absence. <br /> <br />An <br /> <br />employee must request a leave of absence without pay in <br />writing from the Department Director through his/her <br /> <br />supervisor. <br /> <br />If the Department Director recommends <br /> <br />approval, the request will be sent to the Director of <br />Personnel and the City Manager or Assistant City <br /> <br />Manager. <br /> <br />The request must specify the length of the <br /> <br />leave requested. <br /> <br />If approved, a copy of the leave <br /> <br />approval will be placed in the employee's personnel <br /> <br />file. <br /> <br />If an employee does not return to work within <br /> <br />three (3) consecutive working days, after approved leave <br />of absence has ended, the employee shall be terminated <br /> <br />unless another leave of absence is approved. <br /> <br />-58- <br />
