<br />D. <br /> <br />The cost of the examination will be paid by the <br /> <br />city. <br /> <br />All prospective. employees shall be required to <br /> <br />complete a medical history questionnaire before <br />beginning employment. <br /> <br />9. <br /> <br />Aooroval of Aooointment - The supervisor shall submit <br />the documented selection choice in writing to the <br /> <br />Personnel Department for final approval by the <br />Director of Personnel and the City Manager or <br /> <br />Assistant City Manager. <br /> <br />DISQUALIFICATION FROM EMPLOYMENT <br /> <br />The City may refuse to interview, examine or hire any <br />applicant who: <br /> <br />1. <br /> <br />has not met the posted deadline for filing an <br />application; <br /> <br />2. <br /> <br />is found to lack the minimum qualifications necessary <br />to perform the duties of the position; <br /> <br />3. <br /> <br />has made a false statement of material fact on the <br /> <br />City's Employment Application form or supplemental <br /> <br />sheets; <br /> <br />4. <br /> <br />has committed or attempted to commit a fraudulent act <br />at any stage of the employment process; <br /> <br />5. <br /> <br />is physically or mentally unfit for the performance <br />of the duties of the position; <br /> <br />6. <br /> <br />has been convicted of a crime which could conflict <br /> <br />with the position in question; <br /> <br />-7- <br />
