Provide three comparable references for <br />the service. <br />As of June 2008, Wells Fargo has approximately 300 customers with 33,000 cardholders using <br />this service, in addition to 1,000 of our own employees. <br />Which program (authorization mark) does your program use? (Visa, MasterCard, <br />etc.) <br />To provide the City the best solution available, Wells Fargo offers a Visa branded payroll <br />card. <br />Describe the enrollment process. Is enrollment online and does it provide for <br />batched and individual item processing? Are there limitations on the batches? <br />We will provide Employee Enrollment Forms for your employees to complete during <br />implementation. The City then inputs this information into the PayCard Web site. The <br />PayCard service requires cardholder signatures. Each signed form authorizes the City to <br />pay the employee via credits to the PayCard and also to make corrective entries as <br />needed for payroll errors. <br />You may customize the forms we furnish by, for example, personalizing them with your <br />company logo. These forms are then maintained by the City. <br />What are the costs to the institution associated with inactivity (dormancy) or <br />liquidation of stored value amounts to the City? To the end -user? <br />When the City stops loading value to a card, your monthly service fee for it ends. After 90 days of <br />inactivity, the card holder will incur a 5.00 monthly inactivity fee. <br />What are the inactivity (dormancy) levels for the program? What does inactivity <br />trigger? <br />When the City stops loading value to a card, your monthly service fee for it ends. The <br />card remains in your database until your former employee removes all value or closes the <br />account or until the card is inactive for 360 days.
