An applIcatIOn for a restaurant permIt must be accompamed by all of <br />the followmg: <br /> <br />a) A diagram of the floor layout of the busmess, clearly mdicatmg <br />areas where food IS stored, where food IS prepared, and where food is <br />served to customers. <br /> <br />b) A statement of the total seatmg capacity of the busmess, and a <br />statement of the seatmg capacity of the areas of the busmess where <br />food is served. <br /> <br />c) A menu mdIcatmg all food and drInk Items served at the busmess. <br /> <br />I <br /> <br />d) The hours of operatIOn ofthe busmess. <br /> <br />;!.2 The holder of a restaurant permit must submit to the DIrector <br /> <br />a) A complete written update of all mformatIOn reqUIred under thiS <br />sectIOn above each year wIthm 30 days of the annual renewal date of <br />the state T ABC lIcense or permIt for the holder's business... .,....and <br /> <br />b) F or one year after the start of busmess operatIOn under a new <br />CondItIonal Use PermIt. a monthly report on a form approved by the <br />Director. bearing the signature of a Certified Public Accountant. <br />mdicatmg the gross receIPts of the busmess and the portion of the <br />gross receIPts of the busmess derIved from alcoholIc beverage sales. <br />The report must be submitted by the 2Sth day of each month to cover <br />the precedmg month or any applIcable portIOn thereof; and A copy of <br />each report submItted to the TABC that contams an IndIcatIOFl of the <br />portIOFl of revenues of the busmess that are derIved from alcoholic <br />be'lerage sales, wIthm five days of the submIssIOn of the report to the <br />TABC. <br /> <br />I <br /> <br />c) After the first year after the start of busmess operatIOn under a new <br />CondItIonal Use PermIt. a quarterly report on a form approved by the <br />DIrector. bearing the SIgnature of a CertIfied PublIc Accountant <br /> <br />7 <br />
