<br /> I 5 5 U I N G A N IN D U 5 T R I A L <br /> \Y A 5 T E PER M IT <br /> APPLICATION <br /> An applicant for an Industrial Waste Permit shall: <br /> 1. Complete and submit to the Director an Industrial Waste <br /> Permit application form, a copy of which is in APPENDIX <br /> A. <br /> 2. Prepare a Spill Prevention and Control Plan according <br /> to procedures presented in APPENDIX B. <br /> 3. Submit the Spill Prevention and Control Plan and the <br /> appropriate permit fee to the Director. <br /> Written information submitted will be reviewed and plant <br /> facilities inspected by the Manager. Should the Manager <br /> determine that information provided is incomplete or that <br /> proposed liquid waste management plans or plant physical <br /> components are unsatisfactory, the applicant will be notified in <br /> writing of the deficiencies noted and a time schedule specified <br /> to accomplish necessary corrections. <br /> I SSUA.'lCE <br /> When all aspects of application for permi t are deemed to be <br /> in proper order, the Manager will recommend to the Director that <br /> a permi t be issued to applicant. <br /> With application approval and permi t issuance by the <br /> Director, the issuance of permi t sha 11 be published in the San <br /> - <br /> Marcos Daily Record newspaper in the section of Legal Notices. <br /> The permi t, a sample of which is in APPENDIX C, may be <br /> issued for up to a three-year period, with annual review by the <br /> Control Authority to ascertain need for modification of permit <br /> 4 <br />
