<br /> 4 <br /> (b) Information required by the license application form <br /> shall include the name and address of the owner and an <br /> accurate description of the animal to be licensed. <br /> Sec. 3-13. Same-Fees as prescribed. <br /> <br /> The amount of the license fee shall be set by the city <br /> council by separate ordinance from time to time. Fee exempt <br /> licenses may be issued for the following: <br /> (I) Police or sheriff's department dogs. <br /> (2) Dogs trained to work for the blind or the hearing <br /> impaired. <br /> Eligibility for fee-exempt licensing does not relieve the <br /> owner of any responsibility under other provisions of this <br /> article. <br /> Sec. 3-14. Same-Issuance. <br /> (a) Upon compliance with the requirements of this article, <br /> the chief animal control officer or designee shall issue a <br /> durable tag imprinted with an identifying number and the <br /> year of issuance designed so that it can be conveniently <br /> fastened to the animal's collar. <br /> (b) Licenses shall be valid for a period of one (1) year <br /> from the date of licensing. <br /> (c) The owner of each animal licensed under this article <br /> shall keep the tag securely fastened around the neck of the <br /> animal. <br /> Sec. 3-15. Same-Transfer. <br /> (a) No person shall use a license tag for any animal other <br /> than the one for which it was issued. If there is a change <br /> in ownership of a licensed animal, the new owner shall have <br /> the license transferred to the new owner's name. <br /> Application for transfer shall be made in person to the <br /> chief animal control officer. <br /> Sec. 3-16. License Revocation. <br /> (a) Upon petition by the chief animal control officer or <br /> designee, the municipal court judge may revoke an animal's <br /> license after a hearing for anyone or more of the following <br /> reasons: <br /> (1) Impoundment of the animal at the animal shelter <br /> more than three (3) times during any twelve (12) <br /> month period. <br /> (2) More than three (3) final convictions of a person <br /> for violating this article when such convictions <br /> relate to the animal. <br /> (3) Upon a determination that the animal is a vicious <br /> animal. <br /> (b) Upon revocation of an animal's license, the animal <br /> control officer shall immediately notify the owner of the <br /> action in writing. written notification shall be deemed <br /> made when a certified letter, return receipt requested, <br /> addressed to the last known mailing address of the animal's <br /> owner is deposited in the U.S. mail or the notification is <br /> personally delivered to the owner. <br /> (c) Upon the expiration of ten (10) days after written <br />
