<br /> 5 <br /> notification of revocation is delivered as provided above, <br /> it shall be unlawful for any person to keep, maintain or <br /> harbor within the city limits any animal which has had its <br /> registration revoked. <br /> Sec. 3-17 - 3-20. Reserved. <br /> ART1:CLE 1:1:1:. COMMERCIAL ANIMAL ESTABL1:SHMENTS <br /> Sec. 3-21. Permit-Reauired. <br /> <br /> (a) No person shall operate a commercial animal <br /> establishment without a current, valid permit obtained in <br /> compliance with this section. <br /> (b) Application for a commercial animal establishment <br /> permit shall be made on a form prescribed by the chief <br /> animal control officer. Information required by the <br /> application form shall include, but shall not be limited to: <br /> (I) Names and residence addresses of owners. <br /> (2) Trade names under which the establishment is to be <br /> operated. <br /> (3) Location of the establishment. <br /> (4) Type and maximum number of animals to be <br /> accommodated by the establishment. <br /> (5) Whether any owner or employee of the establishment <br /> have ever been convicted of a crime pertaining to <br /> the treatment of animals. <br /> (6) A statement that the owners of the establishment <br /> agree to comply with the provisions of this <br /> article and with all regulations promulgated under <br /> authority of this article by the chief animal <br /> control officer. <br /> (7) A statement under oath by an owner or manager of <br /> the establishment that all information contained <br /> in the application is true and correct. <br /> (c) The permit applied for shall be issued upon successful <br /> completion of an inspection of the establishment by the <br /> chief animal control officer or designee for compliance with <br /> applicable provisions of this article, and upon verification <br /> that no owner or employee of the establishment has been <br /> convicted of a crime pertaining to the treatment of animals <br /> within the three (3) years preceding the date of the <br /> application. <br /> (d) The permit period shall run for twelve (12) months <br /> from the date the permit is issued. Renewal application <br /> shall be made no later than thirty (30) days before the end <br /> of the current permit period. <br /> (e) Subject to approval by the chief animal control <br /> officer, a current valid permit may be transferred incident <br /> to transfer of ownership of a commercial animal <br /> establishment upon payment of the application fee by the new <br /> owner. <br /> (f) Changes in the character of a commercial animal <br /> establishment from one type to another shall be made only <br /> upon a permit being issued for the new type of <br /> establishment. <br />
